The lever test evaluates for ACL rupture. With the knee fully extended, the examiner places a closed fist beneath the proximal tibia at the level of the tibial tuberosity, creating slight knee flexion, and then applies a downward force to the distal femur just above the knee, using the fist as a lever. Before you examine an ACL-deficient knee, always examine the normal knee first. Why? Every person has a unique baseline laxity. Some people naturally have more joint play, more hyperextension, more rotation. If you skip the normal side, you don’t know what is normal for that patient. You may over-diagnose or under-diagnose instability. The normal knee also teaches you: • How relaxed the patient can be • The true end-feel of that patient • Their muscle guarding pattern • Their physiological range
